Insurance & Maximizing Your Benefits  
We are committed to helping you make the most of your dental insurance. Because insurance plans vary, we will provide an estimate of your coverage in good faith, but we cannot guarantee specific benefits due to the complexities of insurance policies.

Important Insurance Information:

• Bring your current insurance card to your first visit.
• If your insurance changes, let us know as soon as possible.
• If an insurance referral is required, please bring the completed form to your appointment.

We file insurance claims on your behalf, with your provider. However, please keep in mind that you are responsible for your account balance, and coverage will depend on your specific insurance plan. If your policy has not paid within a reasonable timeframe, we encourage you to contact us. If you’re insurance company asks for information directly from you, please provide the information as soon as possible to not hold up the claim processing.
